How to provide feedback on the trial Mystery Box delivery service
We want to hear from you and your feedback is the most important part of the trial.
There will be two feedback surveys to complete:
- The first feedback survey will be included in the first Mystery Box delivery.
- Please complete this and put it back into the box with the first Mystery Box items for when our couriers comes to pick them up. Pick up will be at the end of the first 4 week trial period.
- The second feedback survey will be included in the second Mystery Box delivery.
- Again, please complete this and put it back into the box with the second Mystery Box items for when our couriers come to pick them up. Pick up will be at the end of the second 4 week trial period.
Alternatively, you can complete the surveys, take a photo or scan them, and then email them to TCCS.LibraryDelivery@act.gov.au. You will also have the option to fill out a feedback survey online. A link to this will be emailed to you if you become a participant in the trial.
Please make sure you only complete one of these methods for each Mystery Box delivery.
